Skilled Nursing Facility, Wise

Heritage Hall at Wise provides 24 hour nursing coverage. Capacity is 97 beds.  Provides registered nursing care with full range of support services. The care team, along with the assistance and support of the family, develops an individualized care plan tailored to meet the specific needs of each resident. Specialized resident care, looking after the special needs of Alzheimer's patients is also available. Care provided includes recovery bridge care, intermediate care, skilled care and advanced medical care.



We also offer varied resident care programs. American HealthCare provides intermediate care in a traditional nursing home setting, or respite care to take care of a loved one on a short-term basis. Have registered nurses, licensed practical nurses, and trained geriatric nursing assistants. Personnel are trained and prepared for emergencies. Each resident's medical care is supervised by the attending physician who makes routine visits to the home.
