Commonwealth Coordinated Care Plus (CCC Plus) is a Medicaid-managed long-term service and support program that services over 260,000 individuals in Virginia. Enrollment in the program is required for qualifying individuals who benefit from the person-centered care management program. To see if you qualify and enrollment information, click here. 

Transportation is one of your Medicaid benefits. As a CCC Plus member, your transportation to Medicaid services is covered by your health plan, or their transportation broker, if you do not have another way to get there.

You can call Member Services to ask for the transportation phone number, or it may be listed on the back of your Member ID Card. Make sure to call and reserve your transportation at least 5 business days prior to your regular scheduled Medicaid services.
