Hospice

New Century Hospice provides hospice care to patients who have a life limiting illness.  The Hospice team includes physicians who are board-certified Geriatricians and Palliative care certified, registered nurses with hospice experience, medical social workers, chaplain, home hospice aide, volunteers and physical, occupational and speech therapists.

Routine home care, inpatient care, continuous care and respite care are provided to meet the patient's needs. Medical Social Worker helps connect family with community resources. Hospice physician makes house calls; registered nurse manages medications and educates patient and family on end-of-life care. Chaplain will address any spiritual concerns. Volunteer services provides companionship and caregiver relief.

Care for complex patient to include Tracheotomy,  home ventilator, IV therapy, palliative chemotherapy and blood transfusions.

Services available for individuals with mobility impairment, developmental disability or dementia/Alzheimer's disease.

Service is provided wherever the patient calls home.
