Medication management will be provided on a referral basis from other agency programs such
as care coordination, in-home services and nutrition. Client’s will be encouraged to talk with
their physician and pharmacist about their medications. Education will be provided to prevent
misuse of medications and adverse reactions. Clients who need help in securing medications
will be assisted through pharmaceutical companies, discount programs as well as part D
plans. AASC’s eligibility committee will refer clients who are taking excessive number of
medications and who are potentially at high risk for mismanagement, for evaluation and follow
up as indicated by their completed Uniform Assessment Instrument (UAI).
This service is part of the Care Coordination program.
