Who We Are
Professional Health Care Resources offers geriatric care management services. Linda Klingensmith is a Certified Geriatric Care Manager.The goal is to provide comfort and improve the quality of life for the elderly while relieving the stress from caring families.
A Geriatric Care Manager can:
- Conduct care-planning assessments to identify problems, and need for services.
- Coordinate and monitor senior services, such as home care.
- Act as a liaison to families at a distance.
- Offer counseling, support and advocacy.
- Provide information, referral and placement.
- Offer crisis intervention and care management services.
