Who We Are
Senior Advocates provide information, assistance and care coordination services to persons age 60 and over, targeting especially those who are frail, have disabilities or are at risk of institutional placement.
The care coordination includes: evaluating individual needs to remain successful in the community; linking the individual to programs and supports that are needed or requested; and monitoring the supports given to the individual to ensure they are meeting his/her needs.
