Care Management Services

Case management is provided through a grant from the Care Coordination for Elderly Virginians Program. This service supports Loudoun County residents age 60 and older who are at risk of nursing facility placement, older adults who require coordination of multiple services, who live alone and are unable to access services without help, or an older adult whose family or friends live outside of the area, or who are unable to coordinate and oversee services . A case manager can assist an older adult to develop a plan of care based on an assessment of care and service needs.

Case Management services may include:

• Arrangement of service delivery
• Care plan development
• Comprehensive in-person assessment
• Initial eligibility screening
• On-going monitoring of client’s status and delivery of services
• Re-assessment of client's needs over time
• Referral to other eligible programs
• Short-term service coordination
• Supportive counseling
