Care Coordination services are available to people age 60 and over who need help with basic self-care (such as bathing and dressing), and who are unable to obtain these needed services on their own. Services arranged might include transportation, help with bathing, housekeeping, home repairs, relief for caregivers, home-delivered meals, and assistance in applying for public assistance.
Following an in-home assessment of the older person's needs by a care advocate, a plan of care is developed and services are arranged. The care advocate evaluates how well the planned arrangements are working, and makes any adjustments that are needed.
Moderate fees are charged on a sliding scale basis, according to income. People whose income is near or below poverty level are not charged a fee.
Assistance is generally provided in the home of the client.
