Who We Are
This program provides professionally certified care managers to assess older person's living situations and provide cost effective options to meet their needs. Client fees are based on their ability to pay.
Program will research and develop a plan of care, arrange in-home assistance, meals on wheels, assistance with complex paperwork from other agencies, Alzheimer's and Dementia support groups, adult day care, home repair and weatherization, health insurance counseling, legal aid, pharmaceutical program and transportation.
